The activation mapping of showed that AFL was using the isthmus between the right-sided atrioventricular valve and the inferior vena cava and a part of the Fontan circuit The patient had history of double-inlet single-ventricle heart with transposition of great arteries required several surgical repairs including Blalock shunt and classic Fontan procedure connecting right atrium to pulmonary artery.
